Horizon Clubhouse

Horizon House is a community support clubhouse based on the "Fountain House Model". The Horizon House is a psychosocial rehabilitation day program this is intended to support members in all areas of their lives, supporting independence in the community.

We are a program serving mentally ill adults (18 and over) who have been diagnosed with an Axis I form of illness.

The program is open is open Monday and Tuesday, 8:30 a.m. to 5 p.m. and Wednesday 8:30 a.m. to 6 p.m.

The clubhouse social program is available to members Thursday and Friday from 3 p.m. to 7 p.m., Saturday 11 a.m. to 7 p.m. The clubhouse is closed on Sunday, but opened on holidays 10 a.m. To 3 p.m.

We are able to serve mental health consumers with Axis I diagnosis in the Wakefield area. Our catchment area includes: Everett, Malden, Medford, Melrose, North Reading, Reading and Stoneham.

Services Offered

A major component of any clubhouse is its employment program.

We provide employment opportunities through transitional employment placements (TEP), and supported employment placements (SEP). Horizon House also provides housing and educational supports. The clubhouse has a Social Planning and Activity Meeting (SPAM) for evenings and weekends.

Directions to Horizon House

From the North: Follow Route 128 (I 95) South to exit 39 North Avenue. Bear left off the ramp. Follow until you come to a set of lights (located at the end of the lake, on your left hand side). Go strait until you reach a blinking light. Go past that light and take your second left on to West Water Street. Follow West Water Street into Wakefield Center. Proceed through this light. There will be a sign on your left hand side that will say Horizon House. This is not where we are but go past Daddy's Donut on the right (Vernon Street will be on your left.) We are the first building on your left past Vernon Street. If you go by the bowling alley, you have gone to far. The building is pictured above. From the South: Go North on Route 128 until you reach exit 39 North Avenue. Now follow the directions from above.
