Horizon Housing

This residential treatment program serves adults with mental illness who have a goal of living independently in their own apartment.

The program presently serves consumers both in the house as well as in the community.

The program works closely with the Department of Mental Health to help consumers obtain the highest level of functioning. The program is staffed with twenty-four hour a day coverage.

Services Offered

Consumers living in the program are offered various services and are encouraged to be active in their own rehabilitation.

Consumers have treatment plans that address goals such as budgeting, self-medication skills, ADL's and meal preparation.

The program also works with other Human Service agencies for comprehensive consumer treatment.

From Boston: Take Route 93 North. Take exit 37 (Route 95 North). This will be Route 95 North. Take exit 39 (North Avenue) and bear to the right. You will come to a set of lights, continue straight. Anton's Cleaners will be on the left hand side, take this left on to Chestnut Street. The Horizon Housing Program will be on your left hand side, across the street from the Sovereign Bank parking lot. It is the yellow house (pictured above), 5 Chestnut Street. Ring the bell on the left side.
